Where is the mention of metrics? E.g., cycle time? Maybe things are working just fine? Or maybe the team _think_ things are working fine, but the numbers say differently? What is the production defect rate? What is the integration test frequency? What is the integration test coverage (not code coverage)? What is the feature (not story) lead time? What is the actual feedback from customers/users? What is the market-facing success criteria, and how close are we to that?
