A lot of great thoughts here. The only one I have a question about is "self-managing" [team]. What do you mean by that? I hear many Agile coaches use that term today, but do they understand it?

The term "manage" has at least three meaning in the literature. Researchers in Management Science originally defined "manage" to include planning, organizing, leading, and controlling ("controlling" does not mean what it sounds like). How is the term being used here? Are you implying that there are no managers? No team leads?

Leadership is the _most_ important thing. That's why Agile 2 has nine principles related to leadership.
